This form only needs to be completed on your initial sign up.
You do not need to complete it to continue with the monthly subscription, unless your circumstances have changed e.g. your activity levels have significantly increased or dropped. Weight loss does not mean we should reduce our calories. I would advise re-visiting your calorie requirements, if you have had a significant loss and your progress has stalled for a number of weeks despite consistency.
Reminder:
You should not sign up for this plan without consulting with your doctor if you have any health concerns. It is assumed that you are fit and healthy if you do sign up.
